Discounts may apply to <br />large projects. Please call for a job specific quote. <br />Containers/Shipping -Sample containers cleaned to EPA specifi'ation, chain-of- <br />custody forms, seals, coolers, blue ice, and UPS Ground Service shipping charges <br />to clients or same day (local) courier charges are includ d in the listed <br />prices. Client is reponsible for expedited and return s ipping charges. <br />Charges will be assessed for unreturned coolers and contain rs, and returned <br />unused containers. <br />Payment - Due 30 days from invoice date. Invoices not paid w~thin 30 days may <br />be assessed 1.58 interest per month. Minimum billing of $10 .00. <br />Unusual and Difficult Matrices - Premiums and/or increased turnaround times may <br />apply for analyses of unusual''or difficult sample matrices. Client will be <br />contacted prior to proceeding. <br />Holding Times - Appropriate rusif fees may be assessed for sam~les that must be <br />analyzed before the requested turnaround time if the analyses ust be expedited <br />to meet the holding time requirement. <br />Sample Return - All samples remain the property of the client. Hazardous <br />samples and samples unsuitable for municipal disposal will be returned to the <br />client upon completion of the adalysis. Samples placed on hold ill be returned <br />one (1) month from the date of sample receipt. Samples not cons'dered hazardous <br />based on the test results will be disposed of at that time unless advised <br />otherwise by the client. Applicable shipping charges may be i voiced. <br />SamnlinQ Protocol - Clients are urged to comply with EPA pr tocol regarding <br />sample volume, containers, preservation, cooling, sampling pro edures, holding <br />times, field filtering and sample splitting. Evergreen Anal ical assumes no <br />responsibility for the utility of the data in the event that the e protocols are <br />not adhered to. <br />Limit of Liability - Evergreen Analytical, Inc. performs servic~~s in accordance <br />with normal professional standards .for the industry. The total liability of <br />Evergreen Analytical, its officers, agents or employees, aris ng out of or in <br />connection with the services provided, shall not exceed the invoiced amount. <br />Client acceptance of a proposal releases Evergreen Analytical Inc. from any <br />liability in excess thereof, not wittiS~tanding any provision to the contrary in <br />any client purchase order or contract. <br />
