Inspection Report <br />(Fredlund Gravel Pit Project) <br />Page 2 of 2 <br />3. Employee training was not conducted as required in Part LC.9 of the permit. The permittee must develop and <br />implement an employee education program, and identify periodic dates for this training. A summary of the topics to <br />be discussed during [his training must be submitted [o the Division. <br />Facility Insaection <br />Note: Al] Best Management Practices (BMPs) mentioned in the below findings must be installed according to specifications <br />and design criteria outlined in the SWMP. These specifications and design criteria must meet best engineering practice <br />requirements. <br />The facility inspection was conducted during a precipitation event. It was noted during the inspection that significant portions <br />of the site were vegetated (see photograQhs 1 and 21, and that sand and gravel activities at the site were limited to two small pits <br />(see photoeranhs 3 and 4) located in the southwestern and northeastern aspect of the site, respectively. A process water <br />discharge was not observed during the inspection, nor did there appear to be any intent to dischazge process water at tins time. <br />No materials or equipment were present onsite, and no mining activity was in progress at the time of the inspection. <br />4. It was noted during the inspection that the SWMP and Site Map did not reflect current conditions at the site. For <br />example, the Site Map indicates a gravel pit in excess of 20 acres at the site; at the time of the inspection, however, it <br />was noted that sand and gravel activities at the site were limited to two small pits, located on the interior aspect of the <br />site. Also, the S WMP and Site Map identify a sediment pond BMP that is intended to serve as a fihration device for <br />process water from the gravel pit, and for site stormwater run-off directed to the gravel pit, prior to discharge to the <br />agricultural ditch located south of the pit boundary. At the lime of the inspection, however, it was noted that the <br />sediment pond was not installed onsite. The SWMP and Site Map, therefore, did not reflect site conditions noted during <br />the inspection. The SWMP and Site Map must be updated to reflect current conditions at the site. <br />5. It was noted during the inspection that BMPs implemented for the berm located at the eastem perimeter of the site see <br />photo~aph 5) were not identified in the SWMP. A vegetated buffer was noted adjacent to the eastem berm, between <br />the berm and the pit boundary, but this BMP was not indicated in the SWMP. BMPs must be implemented to prevent <br />the offsite discharge of sediment from all berms at the site. If the vegetated buffer is intended to be utilized as a BMP <br />for the berm, it must be identified in the SWMP. The SWMP must be updated to identify all BMPs utilized on site. <br />6. It was noted during the inspection that BMPs were not implemented to prevent the erosion and discharge of sediment <br />from the berm located at the southwest perimeter of the site (see photoeraph 61. BMPs were not implemented to <br />prevent erosion from water run-off from the berms, and no perimeter BMPs were implemented. As a result, there was a <br />potential for sediment discharge offsite, beyond the control ofthe operator at this location. BMPs must be implemented <br />to prevent the offsite discharge of sediment from al] on-site berms. <br />It was noted during the inspection that BMPs implemented for on-site stockpiled material (see photoeraph 71 were not <br />identified in the S WMP. The stockpiled material was located approximately in the middle of the site, with a vegetated <br />buffer between the material and the pit boundaries. All stockpiled material onsite must have BMPs in place to prevent <br />the discharge of sediment from run-on andrun-off water. If the location of the stockpile and the vegetated buffer are <br />intended to be utilized as BMPs for the stockpiled material, they must be identified in the SWMP. The SWMP must be <br />updated to identify all BMPs utilized on site. <br />
