Mr. Kent Gorham
July 19, 2002
Page 2
was lost in the mine during the November 2001 sampling. As a result, the annual sample for
these two wells was not completed until January 13, 2002. Miscommunication with the lab
resulted in the short list of pazameters being analyzed rather than the annual list Measures
to prevent this occurrence from happening again are presented below.
4. Failure to report water level for TH-201, TH-202, and TH-203 for October and November-
Waterlevel measurements for these three wells are presented in the 2001 AHR. Picketwire
requests vacation of this portion of the violation. As for the October water level
measurements, these wells were placed on a reduced monitoring schedule as the Preparation
Plant Facilities were inactivated. Table 1.2-1 of the 2001 AHR has a footnote indicating that
"As the Preparation Plant facilities become active, the Division will be notified in writing
and frequency of monitoring will be increased as indicated". Since the Prep Plant Facilities
remain inactive, there was no requirement to increase the monitoring. Picketwire requests
vacation of this portion of the violation.
In the future, Picketwire will complete the following steps to ensure that all samples are collected
and the appropriate analyses are conducted in the lab:
] . Picketwire will utilize the tables in the Permit as a checklist when sampling is conducted and
in completing the Chain of Custody identifying the analytical list that is sent to the lab.
2. Picketwire will send all sampling results to Greystone as they are completed for a QA/QC
check to ensure all necessary samples have been collected.
3. Picketwire will include Greystone on the distribution list of results from the lab to complete
a QA/QC check to ensure all necessary analyses have been completed.
4. Picketwire will submit a summary of sampling to the Division on a quarterly bases to show
that all analyses are being completed as required. This information will also be included in
the records at the mine site so CDMG inspectors may examine it during the quarterly records
review.
Should you have questions or need additional information concerning the abatement ofthis NOV,
please do not hesitate to call Mr. Ron Thompson or let me know.
~ncerely,
~~ ~
Jerry . Koblitz
Principal
cc Mr. Ron Thompson -Peak
