Minesite Inspection Narrative <br />Inspection Dates April 12 to 16 <br />Mine: New Elk <br />Company <br />Wyoming Fuels Company <br />14300 Highway 12 <br />Weston, Co. 81091 <br />Individuals present during inspection: <br />Ron Thompson, Basin Resources <br />Daniel Hernandez, Division of Minerals and Geology <br />Anthony Waldren, Division of Minerals and Geology <br />Thomas W. Wright, OSM <br />Weather: variable ranging from sunny to cloudy with snow showers <br />Comments <br />This was a complete random sample inspection done to evaluate the State of <br />Colorado's performance in administering their program and to determine if the New <br />Elk Mine was operating in accordance with the program regulations. <br />Paperwork and records was reviewed at the mine office during the afternoon of <br />April 12 and the morning of April 13. The on the ground portion of the inspection <br />was done from the afternoon of April 13 through the afternoon of April 15. <br />A complete inspection of the mine by the State of Colorado had been done from <br />April 5th through April 9th of this year. As a result of this inspection the following <br />violations had been issued: <br />C-93-034 for lack of identification signs and markers <br />C-93-035 for culvert maintenance <br />C-93-036 for construction/maintenance of ditches berms and diversions <br />C-93-037 for unapproved structures <br />C-93-038 for failure to follow the mine plan <br />C-93-039 for adverse impacts to hydrologic balance <br />C-93-040 for lack of contemporaneous reclamation <br />C-93-041 for improper spoil placement <br />C-93-042 for inadequate surface and groundwater monitoring <br />C-93-043 for failure to certify structures <br />C-93-044 for failure to stabilize road embankments <br />
