C-86-068, C-86-069 and <br />C-86-073 <br />Dear Mr. Mills: <br />The Division has not received payment of fixed civil penalties for the above <br />referenced violation. It is important that these penalties be paid so future <br />mining operations you may wish to participate in will not be jeoporadized. <br />Given that you did not request an assessment conference or a Board Hearing on <br />these violations, the penalities have been fixed at the original proposed <br />assessment amount as required. - .. <br />In addition, the violations concerning the sediment pond and the monitoring <br />wells have been terminated.
