Construction activities <br />were performed by Ames Construction Ino. (Ames) and Poly-flex Construction Inc. <br />(Poly-flex) during the period of November 27, 2002 through December 2, 2002. Golder <br />observed and documented construction activities including subgrade prepazation, Soil Liner <br />Fill (SLF) placement, anchor trench excavation, geomembrane deployment, and Drain Cover <br />Fill (DCF) placement. <br />Field earthworks testing was conducted by Golder on SLF placed in the ADR area. Soil <br />Liner Fill field testing of approximately 75 cubic yazds of SLF included monitoring of <br />placement and preparation for geomembrane deployment, testing in-place moisture and <br />density, and performing depth verification testing. Soil Liner Fill placement was performed <br />in accordance with the 2001 Phase IV VLF Construction Specifications. Results and <br />locations of field density and depth verification testing aze presented in Attachment A. <br />' Construction of the ADR azea in accordance with the 2001 Phase IV Construction <br />Specifications required excavation of approximately 98 feet of anchor trench along the <br />southeast side of the extension. <br />' Golder observed and documented the deployment, seaming, defect repair, and testing <br />(Destructive and Non-Destructive) of approximately 3,383 squaze feet of 80-mil single-sided <br />textured LLDPE geomembrane deployed by Poly-flex for the ADR liner extension. As-built <br />construction of the ADR azea geomembrane is shown in Figure 1. Geomembrane was <br />supplied and tested in accordance with the 2001 Phase fV VLF Construction Specifications <br />and was placed to complete the composite liner system.
