(1989 Sup p.) and Permit No. M-77-205 <br />require you to submit 5350.00 in annual fees and an annual report on or <br />before November 29 of each year. As of the date of this letter we- f~ ave not <br />received your annual fees and report. Therefore, you are in violation of the <br />above cited statute and the provisions of your Permit No. M-77-205. Pursuant <br />to Section 34-32-124(71, C.R.S.
