On November 14, 2000, the Board <br />considered your timely compliance with the corrective actions of the violation and issued the enclosed Order to <br />pay a civil penalty. <br />We strongly advise that you read this document carefully, since it contains provisions which must be satisfied by <br />specific dates. DMG records indicate that on November 14, 2000, we received payment for [he civil penalty, in <br />the form of check no. 2474, in the amount of $629.30.
