Preliminary <br />review of the information received determined that the following items must be received before the Division can <br />consider your apglicataon as being submitted and technical review can begin: <br />APPLICATION FORM - Responsibilities as a Permittee Page 5 and 6: Please complete Items 1 <br />through I O and return these two pages to the Division. <br />EXHIBIT O -Proof of Mailing of Notices to County Commissioners and Soil Conservation <br />District (Rule 6.4.17): Please submit proof of actual delivery or proof of mailing by CertiSed Mail, <br />retbm receipt regbosted, of Notice bf the Application to the local Board of County Commissioners and <br />the Board of Supervisors of the Conservation District. The green certified mail receipts submitted do <br />not have the post office stamp indicating the notices were actually mailed. <br />_ EXHIBIT S -Permanent Man-Made Structures (Rule 6.4.19):. You must provide information _ <br />sufficient to demonstrate that the stabIlity of any struchues located within two hundred (200) feet of the <br />operation or aff®cted land will not be adversely affected. The Division cannot accept fhb statement fliat <br />Chaffee County will not adversely affect the stability of any significant, valuable or permanent man- <br />made structures. You must either provide information sufficient to demonstrate that the stability of any <br />structures win not be adversely affected or an agreement with the owners of the structures. The <br />Division can accept a signed statement from Chaffee County stating that you wdi stay at Lease 200 feet <br />away from the structures umil an agreement is submitted to the Division. <br />Your application will not be considered submitted until the information listed above is received and found <br />sufficiem to begin our review. A decision date wi11 be established 90 days from the date of receipt of all of the <br />ragitested inforhation. Additionally; if you have already publShed notice you will need to republish notice, <br />after the Division considers the application submitted. This notice must be published once a week for four (4) <br />consecutive weeks, starting within ten (10) days of the date your application is considered submitted. We will <br />notify yob wlien you should initiate republication of your notice. The Snot date for receiving comments is the <br />20th day after the fourth publication or the ne3d regular business day. <br />
