Preliminary review of the information received determined that the following items must be received before the Division can consider your apglicataon as being submitted and technical review can begin: APPLICATION FORM - Responsibilities as a Permittee Page 5 and 6: Please complete Items 1 through I O and return these two pages to the Division. EXHIBIT O -Proof of Mailing of Notices to County Commissioners and Soil Conservation District (Rule 6.4.17): Please submit proof of actual delivery or proof of mailing by CertiSed Mail, retbm receipt regbosted, of Notice bf the Application to the local Board of County Commissioners and the Board of Supervisors of the Conservation District. The green certified mail receipts submitted do not have the post office stamp indicating the notices were actually mailed. _ EXHIBIT S -Permanent Man-Made Structures (Rule 6.4.19):. You must provide information _ sufficient to demonstrate that the stabIlity of any struchues located within two hundred (200) feet of the operation or aff®cted land will not be adversely affected. The Division cannot accept fhb statement fliat Chaffee County will not adversely affect the stability of any significant, valuable or permanent man- made structures. You must either provide information sufficient to demonstrate that the stability of any structures win not be adversely affected or an agreement with the owners of the structures. The Division can accept a signed statement from Chaffee County stating that you wdi stay at Lease 200 feet away from the structures umil an agreement is submitted to the Division. Your application will not be considered submitted until the information listed above is received and found sufficiem to begin our review. A decision date wi11 be established 90 days from the date of receipt of all of the ragitested inforhation. Additionally; if you have already publShed notice you will need to republish notice, after the Division considers the application submitted. This notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date your application is considered submitted. We will notify yob wlien you should initiate republication of your notice. The Snot date for receiving comments is the 20th day after the fourth publication or the ne3d regular business day.
