Long <br />Dmsion Dueaor <br />As requested, I have reviewed the proposed assessement provided by me on July 22, 1993 for <br />the above-referenced Notice of Violation (NOV). The history component contained an error. <br />Two NOV's existed, and were final, as of July 22, 1993. These were C-93-005 and C-93-055. <br />I apparently wrote the permit number instead of the NOV number for NOV C-93-055, as I <br />rendered my proposal. <br />CO C-93-054 also existed at the time of the assessment, and this was missed. Three <br />explanations are possible: <br />I failed to see the CO file; <br />2. The CO file was not located in the public file box, <br />since many files had been separated and taken to <br />specialist offices; <br />3. The payment receipt received July 7, 1993, had not <br />yet been filed and, as such, 1 assumed that the <br />appeal process was still pending. <br />It's possible I missed it, but I try to be very careful in checking the public file boxes.
