Henderson Mine. 303/569-3221 ext. 1287 <br />Summary and Resolution of Interaction: <br />On October 6, 2003 Anne Beierle at the Henderson Mine telephoned in a report that the geomembrane in Pond <br />1.2 had been ripped. Pond 1.2 was approved through technical revision TR-008 to be an Environmental <br />Protection Facility for disposal of dewatered high-density water treatment sludge generated at the Urad water <br />treatment plant. The rip occurred when a load of sludge was dropped onto the liner at a location where the liner <br />was tightly stretched and there was an air pocket beneath the liner (between the geomembrane and the substrate). <br />The rip is high on the side slope of the pond so there is substantial freeboard between any sludge or liquid in the <br />bottom of the pond and the location of the liner damage. No sludge or liquid escaped the containment of the <br />liner as a result of the damage. It was agreed that Henderson would file a written report in accordance with Rule <br />8 of the Mineral Rules and Regulations once the liner had been repaired. <br />On November 25, 2003 Tony Lucero at the Henderson Mine left a voice message stating that Colorado Lining <br />Company, the original liner installer, had been contacted to repair the liner. Colorado Lining recommended that <br />the repairs not be undertaken in the winter conditions at the mine. The repairs will be made in the spring of <br />2004. Tony reiterated that the rip in the liner does not compromise Pond 1.2 as an Environmental Protection <br />Facility due to the location of the rip high on the side slope of the pond. <br />The Division of Minerals and Geology (DMG) agrees that the rip repairs should be done in the spring under <br />suitable weather conditions. Once the repairs are completed, Henderson must provide DMG with a report <br />describing the repairs and documenting the seam and liner integrity testing conducted.
