The Board ordered the permit <br />revoked and the financial warranty forfeited. <br />The Operator explained that due to medical emergencies in his family and <br />miscommunication among family members, the Operator did not receive notice of the civil <br />penalties or permit revocation proceedings. It was only after Division staff contacted a <br />friend of the Operator that he became aware of the penalties and permit forfeiture. He did <br />not intend to let the permit lapse or avoid paying annual fees. <br />4.
