June 24, 2003 -2- 003-2203.7 <br />• presented in this report are current as of June 2003, which represents completion of the Phase Nb <br />PSSA, as detailed in this report. <br />1.2 Construction Quality Assurance/Quality Control <br />Golder provided CQA and CQC monitoring and testing services, and prepared this report describing <br />the monitoring and test results for the Phase Nb PSSA. As-built drawings and supporting CQA and <br />CQC monitoring and testing documentation are presented in appendices attached to this report. <br />1.2.1 COA Monitoring of Earthworks <br />Golder provided CQC testing and CQA testing and monitoring of earthworks for the Phase Nb <br />PSSA. CQA testing and monitoring of earthworks was provided for in-place materials in accordance <br />with the Golder Earthworks CQA Plan. The earthworks material specifications for this project are <br />summarized in Table 1. CQC earthworks testing was provided to support the contractor during the <br />manufacture of materials and prior to material placement. The CQC test results are not included in <br />this report because these tests were intended to assist the contractor in materials preparation rather <br />than record testing. <br />Golder provided CQA laboratory and field testing, and monitored earthworks construction activities <br />as follows: <br />Golder visually monitored and documented the remediation of underground <br />workings within the Phase Nb PSSA footprint, including verification that Ames <br />excavated underground workings to the minimum specified dimensions and then <br />installed Coarse Shafr Backfill (CSB), Concrete Plug (CP), Cemented Rockfill <br />(CR), Fine Shaft Backfill (FSB), Select Structural Fill (SSF), Structural Fill (SF), <br />and geogrid, as specified on the Issued For Construction (IFC) drawings. <br />• Golder monitored construction of the Primary (PU) and Secondary Underdrains <br />(SU), including verification that Ames excavated trenches to minimum required <br />dimensions and grades, and placed materials as specified. Golder performed <br />CQA laboratory testing of grain size, Atterberg limits, and moisture content of <br />the Underdrain Fill (UF) and SSF placed in the Underdrains. <br />• Golder monitored construction of the Leak Detection Trench (LDT), including <br />verification that Ames excavated the trench to minimum required dimensions and <br />placed materials as specified.
