<br />• signatures involved in chain of possession, and <br />• inclusive dates of possession. <br />B-1.10.5 Sample Shipment <br />Properly-completed Chain-of-Custody documentation is prepared and placed inside the shipping <br />container in a waterproof plastic bag, so that it will be itrtmedia[ely apparent to the laboratory personnel <br />receiving the container, but will not be damaged or lost during shipping. The shipping containers will be <br />sealed, so that it will be obvious if the seal has been tampered with or broken. <br />A Chain-of-Custody form will be completed for each shipping container. Each time the sample bottles or <br />samples change hands, both the sender and the receiver need to sign and date the Chain-of-Custody form.. <br />The completed Chain-of-Custody becomes a permanent record of the sample handling and shipping. <br />Samples are transported by sampling personnel directly to the laboratory or are shipped via an express <br />carrier (i.e., Federal Express). Shipping instructions and receipts provided to the sender and addressee by <br />the shipper will become part of the permanent record. Such instructions and receipts should be resumed <br />to [he facility. <br />B-1.11 Sampling Records <br />To provide complete documentation of sampling, detailed records will be maintained during sampling. <br />These records will include the information listed below: <br />• sample location (facility name); <br />• sample identification (sample area name and sample number); <br />• sample location map or detailed sketch; <br />• date and time of sampling; <br />• sampling method; <br />• field observations of: <br />- sample appearance (i.e., color, suspended sediment), and <br />sample odor; <br />• weather conditions; <br />• sampler's identification; and <br />• any other information which is significant. <br />Surface-water sampling information will be recorded on the surface-water sampling record (Figure B-l). <br />
