INFORMATION ABOUT THE VIOLATION <br />RULE 5.04.3 (1) <br />NOTICE OF VIOLATION NO. C-93-49 <br />SUBMITTED BY: ENERGY FUELS MINING CO. <br />on APRIL 30, 1993 <br />PERMIT C-82-055 <br />FACTS SURROUNDING THE VIOLATION: <br />1. Watershed Diversion Ditches Nos. 1 and 2 <br />Io their current conditions, bath ditches are open and able to carry designed flows. There are no <br />indications of breeching along tltese ditches. There is some downcutting, averaging about one foot deep, <br />occuring for a distance of approximately 300 feet in Section B, Diversion No. 2. <br />2. Catchment Basins <br />The two permitted catchment basins below the refuse pile contain sediment which is at or below the <br />elevation of the lowest discharge pipe in both embankments. Although sediment removal would improve the <br />system, both catchment basins are technically in compliance. <br />II. ABATEMENT <br />Energy Fuels is currently performing maintenance on the mine facility drainage control system; the <br />following items Gave been corrected: <br />+ Refuse collection ditch on west side of refuse pile -reconstructed during week of April 19 <br />+ Low drainage area in front of No. 1 (east) mine -corrected and drained during week of April 12 <br />+ Low drainage area in front of Ho. 2 (west) miae -corrected and drained on April 23 <br />+ Downcut section of Watershed Diversion Ditch No. 2 - 75% regraded by April 30 <br />+ Lower end of Watershed Diversion Ditch No. 1, Section A -regraded on April 29 <br />
