O'Brian of Environment, Inc. has forwarded to me your letter dated February 29, <br />2004. Steve and I have worked together to prepare the amendment application, and we have <br />agreed that I would respond to your letter, since I have some of the information which you have <br />requested. Following are responses to the issues raised in your letter: <br />• We have revised Page 2, of the Application Form, to include completion of Item 9. <br />We are including the original initialed pages 5 and 6 of the Application Form, which were <br />inadvertently omitted from the initial submittal. <br />• Steve O'Brian informed me that he had discussed the request regarding Exhibits C and <br />D-1 with you. Map Exhibit C was submitted on 8/31/79 as the "AFFECTED LANDS <br />MAP." At the same time Exhibit D-1, "MINING PLAN MAP" was submitted. On 1/30/92 <br />the "MINING PLAN MAP" was revised and submitted in a Technical Revision. The <br />maps have not changed since those previous submittals and therefore are not included <br />in the current Amendment Application.
