O'Brian of Environment, Inc. has forwarded to me your letter dated February 29, 2004. Steve and I have worked together to prepare the amendment application, and we have agreed that I would respond to your letter, since I have some of the information which you have requested. Following are responses to the issues raised in your letter: • We have revised Page 2, of the Application Form, to include completion of Item 9. We are including the original initialed pages 5 and 6 of the Application Form, which were inadvertently omitted from the initial submittal. • Steve O'Brian informed me that he had discussed the request regarding Exhibits C and D-1 with you. Map Exhibit C was submitted on 8/31/79 as the "AFFECTED LANDS MAP." At the same time Exhibit D-1, "MINING PLAN MAP" was submitted. On 1/30/92 the "MINING PLAN MAP" was revised and submitted in a Technical Revision. The maps have not changed since those previous submittals and therefore are not included in the current Amendment Application.
