<br /> <br />NOV - 90 - 46 <br />OCTOBER 16 - Silt fence inspected, documentation for NOV begun at <br />approximately 1700 hours. <br />- Caught Bill Green at 1745 hours, informed him of the <br />need to repair silt fence, conveyed instructions to <br />him in the presence of David Berry. Instructed him to <br />meet me at the silt fence at 0630 hours October 17. <br />OCTOBER 17 - Arrived on-site 0640 hours, conveyed specific <br />instructions for repair, provided diagrams to follow <br />for proper installation. Due to problems with support <br />stakes, it was decided to replace those stakes with <br />re-bar stakes (after checking with Mr. Berry). <br />Progress checked 1730 hours, and after further <br />assessment the decision was made to remove the <br />existing fence and completely reinstall the entire <br />structure. Removal of fence continued until <br />approximately 2000 hours. <br />OCTOBER 18 - Met with Bill Green on-site at 0630 hours. Removal of <br />existing structure completed 1030 hours, dirtwork <br />preparation for proper key-in begun. <br />At 1500 hours Mr. Berry assessed anchor ditch <br />preparation, and indicated that the trench should be <br />adequate. Anchor trench completed approximately 2000 <br />hours. <br />Snow mixed with rain fell overnight, and continued to rain and snow <br />during the 19th. <br />OCTOBER 19 - 0700 installation begun; roof bolts deemed best <br />alternative for support stakes. Installation continued <br />during rest of day, with the fence having been keyed <br />in and support stakes installed for a portion of the <br />area. <br />
