A complete quarterly inspection of the Roadside Portals Mine was conducted by Dan Mathews <br />of the DMG Graitd ,luttction Office. The inspection was conducted on the afremoon of <br />December I S. beginning at 1:30 p.nt, and continued on December 22, beginning at 9:00 a.m. In <br />addition. Pond 1 ~ +~as inspected on the afternoon of December ~, 1997, to check abatement of <br />NOV C-97-00 L .-u the time of the NOV abatement inspection. Pond l2 had been completed and <br />certified. and contained minewater below the level of the primary spillway. As a result, a <br />[ennina[ion notice liir NOV C-97-001 was issued at [hat lime. Larry Reschke of PCC was <br />present dwing the inspection on December 18. and December 22, and Bob Blue was present <br />during the NOV ahatement inspection on December 5. <br />Availability of Rerurds <br />Required records ~~ere re+iewed in the mine office, and in general were corplete and up to date. <br />It was noted that the insurance certificate on file had an expiration date of December 31, 1997. A <br />renewed certificate ++ill need to be placed on t7le and submitted to the Division. Also, it was <br />noted that the Annual H+drology Repurl (AHR) for 1996 has not yet been completed. This was <br />discussed wills tltc operator. The AHR due date is March 31. for the preceding water year. The <br />Division has granted n+'u extensions to the submittal deadline for the 1996 AHR, in part to allow <br />[he operator ut fo~u> iim~ .tnd etliirts on a major permitting action, the recently approved PR-02. <br />Extension beron~l I;utuar. ; I. 1995..yill not be grained. and failure to submit the 1996 AHR by <br />that date nut+ rexih in an rnli~rcement action. <br />Pond 12 certilicmiun ;uurment +vas provided at the time of the inspection. Second, the plan view drawing on <br />pale RSN-~17 ul .\ppcndix I3-2 depicts the mine discharge line entering the pond at the northeast <br />corner. l he Intr artuall~ enters the pond at its southeast corner, and the certification statement <br />should be antend~.l iu r.ll,ct this minor drmee. <br />Roads <br />Most roads were in gaud condition. with surfaces and ditches maintained since the high intensity <br />rainstomts which occurred in portions of the permit area in September. Maintenance of the inlet <br />to the 72" road cul~ cn on Coal Gulch, between CRDA-1 and Pond 11, mentioned in the previous <br />compete inspection. Itad trot yet.been completed. Flood Flows in Coal Gulch had deposited rock <br />-?:::. aril sediment, and dislodged iiprap,immediately above the culvert inlet.'
