Long <br />Division Director <br />On April 15, 2002, the Division of Minerals and Geology received your response to the Division issuing a new <br />permit for the site currently issued in the name of Westem Slope Flagstone, Permit No. M-1996-076. The Division <br />has not processed your request due to the fact that your letter requested that the Division ptrt the pemut in your <br />name and the Company Name as Western Slope Flagstone and change the site name to Cameo Gravel Pit. The <br />Division cannot put the permit in both your name and Western Slope Flagstone. It must be either put in your name <br />or Western Slope Flagstone. Currently the pernut is issued to Western Slope Flagstone and the site name is <br />Western Slope Flagstone Quarry No. 2. The Division reviewed the file and we feel that the pemtit should have <br />been issued to "Rudolph Fontanari" with the site name being Western Slope Flagstone Quarry No. 2. The Division <br />feels that the pemlit should be issued to Rudolph Fotrtanari but we cannot issue a new permit without your consent. <br />Please let the Division know if you would like the permit reissued in your name. The site name can be changed <br />during the Conversion process with another letter from you requesting that the site name be changed to Cameo <br />Gravel Pit.
