<br />01/15/96 RCO called Devereaux, to ask where were application
<br />materials. Devereaux thought they were sent, and said he
<br />would contact Vasquez to resolve.
<br />01/26/96 RTB letter sent to applicant, for failure to meet date
<br />for obtaining permit as set forth in Board Order.
<br />02/29/96 RCO called Devereaux, to ask where application materials
<br />were. Devereaux said he would contact Vasquez.
<br />02/29/96 Vasquez called RCO, to say that all application materials
<br />would be sent "next week." RCO asked whether original
<br />lost file was ever located, as was told "yes." RCO asked
<br />whether applicant needed anything copied from Division's
<br />copy of file, and was told "no."
<br />03/08/96 Vasquez called RCO, to request copy of all application
<br />file materials. Said this was to complete the unfinished
<br />application, and to determine who had started application
<br />but never finished it.
