<br />Part 1: Failure to limit the maximum Hounds of explosives detonated within any 8-millisecond time roeriod
<br />to the amount allowable. Failure to record the actual maximum weight of explosives detonated within an 8-
<br />millisecondtime period. Part 2: Failure to record in blast records the depth of some ofthe holes, and thus,
<br />in turn, failure to record the total weight of explosives in those holes. Repeated deficiencies in part for blasts
<br />of 11/2/02. 2/2!03, 2/3/03, 2/6/03, 2/12!03, 2/13103, and 3(19/03. Part 3: Failure to maintain an annual
<br />calibration of a seismograph used to periodically monitor airblast. Part 4: Failure to record, on airblast
<br />records, the name of the person taking seismograph airblast readings and the name of the person analyzing

<br />PORTION OF THE OPERATION TO WHICH THIS NOTICE APPLIES:
<br />Part 1, Blasting -The calculated allowable weight of explosives detonated within an 8-nllisecond time
<br />period for the 4:04 PM, February 16, 2003 blast, at the east side of the East Pit, was 3,303 pounds. The
<br />maximum weight of explosives detonated within an 8-millisecond time period recorded on the blast record
<br />was 2,970 pounds. The reconstructed maximum weight detonated (reconstructed) was 9,135 pounds.
<br />Part 2, Blasting, blast record keeping -The depth of some holes detonated in the 4:04 PM, February 16,
<br />2003, blast (east side of the East Pit), and thus, in turn, the total wei t of explosives in those holes were not
<br />included in the blast record. Repeated deficiencies in part for blasts of 11/2/02, 2/2/03, 2/3/03, 2/6/03,
<br />2/12(03, 2!13!03, and 3/19!03.
<br />Part 3, Blasting, Calibration of seisomograpb -The seismograph used to periodically monitor for airblast
<br />was out of annual calibration for the period December 14, 2002 to Apri129.2003.
<br />Part 4, Blasting, airblast record keeping - No airblast record inspected indicated the name of the person
<br />taking the reading and the name of the person analyzing the airblast record. The record of August 30, 2003
<br />carried the signature of Colowvo Blast Engineer Mathew Lengerich.
<br />STEPS NECESSARY TO ABATE VIOLATION (REMEDIAL ACTION):
<br />Part 1: Implement a procedure, by submission of revised pages to the Permit Application Pack~e, that
<br />rorovides for accurate records with regard to the maximum weight of explosives detonated within any 8-
<br />millisecond time period.
