The violation was issued for the alleged failure to <br />obtain all of the required samples from three spoil wells during the fall of 1997 and spring of <br />1998. The wells subject of the NOV are 026-SP-1, 026-SP-2 and 026-SP-3. <br />As is shown on the attached pages, CYCC sampled the wells in accordance with its approved <br />hydrologic monitoring plan. The confusion on the lack of sampling results from the data not <br />being included in CYCC's hydrologic monitoring database. Upon completing a search of the <br />field notes, chain of custody and lab sheets, it was determined all of the data was collected for <br />these wells. The data was not summarized and included in the AHR. The attached page 4 and <br />Tables 14 through 16 can be passed onto your staff for inclusion in the 1998 AHR. The other <br />attachments are the lab data sheets, chain of custody and field notes demonstrating that the <br />sampling was completed as specified in the permit. <br />In order to reduce the possibility of this error occurring again the process for locating missing <br />data was reviewed with my staff.
