<br />Siloam Clay Mine <br />(Permit No. M-77-326) <br />1. Complaint received on 12/9/96 from Joseph O'Brien of Red Creek <br />Ranch. <br />2. Tom Schreiner of DMG conducted site inspection on 12/27/97 and <br />found mining disturbances outside the permitted area. <br />3. DMG sent Reason to Believe letters to Adience, Inc. and Siloam <br />Stone Company on 1/6/97. <br />4. Survey received on 1/21/97 from Valley Surveying and follow-up <br />field inspection on 1/24/97 indicate a total disturbance of 22.0 <br />acres of which 15.5 acres are disturbed outside the 9.9 acre permit <br />boundary. <br />5. Review of permit file documents indicates that permit applicant <br />made commitments to include topsoil stockpiles, product stockpiles, <br />and site access roads within the affected land boundary and that <br />all disturbed lands will be reclaimed. <br />6.
