If you agree with these settlement agreements, please <br />sign them and return them to our office as instructed on the form. The form <br />must be signed within ten days of the above date. If you disagree with either <br />agreement, do not sign the form. The Division will fix the civil penalty as <br />originally proposed and you may appeal that decision to the Board as outlined <br />in the regulations, Rule 5.04.4.
