Dan wn^-~^~t~ and I discussed
the apparent wnfirsion regarding fire mailing address the insurance company has, the
necessity to modify the htnguage oa the insurance binds to satisfy CDMG roquiremerks,
and the rrquitement for a valid signature, I plan to contact the agent you have been
dealing with in the next several days to rectify these eottcems. tLs I will be out of the
office next week, aad the egret is not in Colorado, I anticipate the reed for additional
time to produce the required documem. I[ is my understanding that the mine bas nor been
without liability insurance coverage, but that documentation is not present and sufficient.
I will obtain that documentation and provide it to CDMG by June 15, 2000 if that
extrusion date is acceptable.
NOV CV 2000-003 was issued for failure to maintain copies of records sad reports. If
acceptable w the Divisioo, I would request an extension of the abate~at deadline to July
10, 2000 (90 days from issuance of the NOV). As you are aware, some of the requested
information may not be avat7able, sad I need to determine that which is and that which is
not I will also need some time to prspere the requested annual reclamation report and
annual hydrologic report. I am also in the process of updating the required records and
