You are hereby notified, pursuant to C.R.S. 34-33-123(8)(b), that the Mined Land
Reclamation Division ("the Division") of the State of Colorado, Department of
Natural Resources, proposes to assess a civil penalty against you in the a.~nount of
51,000.00.
If you wish to request a conference at which `he proposed cenalty may be revie~aed,
you may do so by so indicating on and recurnir,G to the Di iision the attached page
of this notice. Your request should be completed in the space provided and
returned to the Division within ten days after your receipt of this notice.
If you fail within ten days after service of this notice upon you to request such
conference, the Division will order the penalty fixed at whatever amount it deems
appropriate based upon the factors referred to in C.R.S. 1973, 34-33-123(8)(d).
