Cattany <br />Re: McCoy Aggregates, Permit No. M-1981-088, Incomplete 112 Construction Materials Division DlffdOr <br />Reclamation Permit Amendment Application Package, AM - 2 / Natural Resource Trustee <br />Deaz Mr. Rozelle: <br />On February S, 2004, the Division of Minerals and Creology reviewed your 112 Construction Materials Reclamation Permit <br />Amendment Application package for the McCoy Aggregates site, Permit No. M-1981-088. Preliminary review of the <br />information received determined that the following items must be received before the Division can rnnsider your application as <br />being submitted and technical review can begin: <br />APPi.iCATTON FORMS The site name on the application and throughout the exhibits is shown as McCoy Pit. The <br />site name was changed in the Succession of Operators application between Western Mobile Northern, Inc. to Lafarge <br />West, Inc. by Mr. Gary Tuttle, of Lafarge West, Inc., to McCoy Aggregates. The Division requests that Lafarge West, Inc. <br />submit a letter stating what [his mining and reclamation operntion is to be called. <br />FXAiRiT R _ Prnnf of Filjpg with ('nnnty ('lerk and Recorder (Role 6-4-tRl: You must submit an affidavit or <br />receipt indicating the date on which the application was placed with the local County Clerk and Recorder. The <br />receipt for mailing does not give the date that the County Clerk and Recorder actually received the amendment <br />application for public review. <br />Your application will not be considered submitted until the information listed above is received and found sufficient to begin <br />our review. A decision date will be established 90 days from the date of receipt of all of the requested information. <br />Additionally, if you Have already published notice you will need to republish notice, after the Division considers the <br />application submitted. This notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days <br />of the date your application is considered submitted. We will notify you when you should initiate republication of your notice. <br />The final date for receiving comments is the 20th day after the fourth publication or the next regulaz business day. <br />You have silly (60) days from the above date to subatit all necessary documents that the Office needs for an application to be <br />considered filed. II; at the end of the sixty day period, the application has not been determined to be filed with the Office, the <br />Office shall deny the application and terminate the application file. <br />This letter shall not be construed to mean that there are no other technical deficiencies in your application.
