Cottony <br />Division Director <br />Natural Resource Tm9ee <br />On April 13, 2005, the Division of Minerals and Geology received your 110 to 112 Construction Materials Reclamation Permit <br />Conversion application for the Grand Gravel Pit, which is located in Grand County. All comment and review periods began on <br />April 13, 2005. The decision date for your application is scheduled for July 12, 2005. <br />The Division is returning the signamre page of your application to have your Notary Public properly notarize your application <br />form. The Notarization should read: <br />The foregoing instntment was acknowledged before me [his 30Th day of March 2005 by Richard R. <br />('nr and Thnmac F- Cnrrv ag Owner and Operator Of Grand Gravel. <br />You will need to place a copy of this page with the application form a[ the Grand County Clerk and Recorder's Office. Retum <br />both original signature pages to the Division as soon as possible. <br />Within ten (10) days after submittal, you must publish notice in a newspaper of general circulation in the locality of the proposed <br />mining operation once a week for four (4) consecutive weeks. Immediately after the first publication, mail a copy of the notice to <br />all owners of record of surface and mineral rights, holders of any recorded easements, and all owners of record of lands that are <br />within 200 feel of the boundary of the affected land. Ptoof of notice and mailings, such as Certified Mail -Return Receipt <br />Requested, must be submitted to the Division of Minerals and Geology prior to the decision date. Additionally, any changes or <br />additions to the application on file in otu office must also be reflected in the public review wpy which has been placed with the <br />Grand County Clerk and Recorder. - <br />The Division is reviewing your application to determine whether it is adequate to meet the requirements of the Act.
