M.D. <br />LOBATO, Fidel Ref: Leo W, Lloyd, M.D.. <br />02-10-83: This patient is definitely improved since his recent <br />hospitalization. He is having much less discomfort in the back and leg. He <br />still is having some numbness but this seems to be decreasing. One problem <br />he's developed is difficulty sleeping end we are going to give him a prescrip- <br />tion for some Ativan to see if that will improve that situation. He is to <br />continue nis rigorous prograir ~, ~.:~:: ~.,.r~ises; we w~1.1 want to see him <br />again in about three weeks. <br />cc: <br />03-21-83: This patient's <br />still having some problem into the lef <br />ing on his toes, but he feels streng`:h <br />motion is also improving, albeit slow. <br />patient PT would speed things up and I <br />DLF/t <br />back is slowly improving. He is <br />t leg, especially difficulty walk- <br />is improving. His range of back <br />He wonders if some additional out- <br />would tend to agree. We will make <br />arrangements for this but of coursete is to continue on a diligent program <br />of home exercise. To be rechecked again in about a month. <br />cc: <br />DLF/ t <br />06-13-83: Patient does seem to be slowly improving. He has <br />not been getting any PT lately and does feel that he has stiffened up some <br />since discontinuing the therapy, ar.d today is complaining of some soreness <br />between the shoulder blades as well as the low back. I would like him to <br />continue with~his PT at least once or twice a week. I've also suggested <br />that he try Motrin, 600 mg, t.i.d. We will check him again in a month <br />to see how things are progressing. <br />cc: <br />DLF/t <br />11-25-83: This patient's back is better but whenever- he tries <br />to do anything in the way of physical activity such as working about his home <br />or mine he immediately experiences a flareup in the back pain. He hasn't been <br />taking therapy. I think probably he can get by merely with home exercise and <br />Motrin. I don't feel he is ever going to be a candidate for resumption of <br />his strenuous activity that he was previously doing, and I think we're look- <br />ing at long term disability in this individual unless he can find something <br />very light to do.
