III IIIIIIIIIIIIIIII <br />999 <br />Notice of Violation No. C-97-002 <br />Facts Surrounding Violation <br />Permit application requires placing refuse in maximum 2' lifts. <br />Terramatrix report states that 1' of fines is placed, followed by a 2'-4' lift of coarse <br />materials, for total lifts of 3' to 5'. <br />Permit application requires compaction of each lift to 90% density prior to placement of next lift. <br />TerraMatrix report indicates entire 3'-5' lift being compacted at once. <br />Quarterly inspection reports show compactions as low as 70.4%. <br />Quarterly reports are inadequate to determine the number and location of compaction tests, and <br />do not include observations of lift thickness. <br />Division letter dated July 19, 1995 noted the reports should also include such information <br />as site conditions, lift thickness, test locations, testing methods, and compaction test <br />results. Most recent reports include only a range of compaction values for an unknown <br />number of tests. <br />Events L.eadinQ to Issuance of Notice <br />July 1995 Division letter regarding testing and reporting problems. Problems noted not corrected <br />to date. <br />October 1995 Division letter noting compaction problems. EFCI commits to evaluation by <br />November 30, 1995. <br />July 1996 letter from Division reiterating concerns, notes EFCI still has not addressed concems. <br />EFCI finally submits report on pile evaluation on January 27, 1997. Report notes construction of <br />waste pile is not in compliance with Rules and permit. <br />
