Exhibit F -Reclamation Plan Mag <br />16. Please revise the Reclamation plan map to illustrate how the site will look after reclamation is <br />complete. Ensure the map details are consistent with the reclamation plan details. <br />Exhibit G -Water Information <br />17. Please address the requirements of Rule 6.4.7(3), (4), and (5). <br />Exhibit H -Wildlife Information <br />18. Please provide the information required by Rule 6.4.8(1)(d). <br />Exhibit J -Vegetation Information <br />19. Please provide the information required by Rule 6.4.10(1)(a) and (2). <br />Exhibit L -Reclamation Costs <br />20. Please indicate estimated equipment to be used, push/haul distances, and grade factors for each <br />task. Ensure the maps corroborate these figures; i.e. stockpile volumes, distances to final <br />disposition, grades, etc. <br />21. Please revise this exhibit to account for required changes made to the reclamation plan in <br />addressing the comments above including overburden placement and grading, topsoil application <br />and tilling, and revegetation of all affected areas. <br />22. DMG will calculate an estimated reclamation cost estimate using the information provided in your <br />response. <br />Exhibit O -Owners of Surface Affected Lands and Subsurface to be Mined <br />23. This exhibit Should include only the owners of surface and subsurface within the affected area <br />boundary. The adjacent landowners information is required on Exhibit C maps and in exhibit S <br />for all those with permanent man made structures on or within 200 feet of the affected area <br />boundary. <br />Exhibit S -Permanent Man-made Structures <br />24. If Unit 11 is to be considered as affected area and occupies all of the permit area not included in <br />other Units, then there appears to be several permanent man made structures and utilities within <br />200 feet of this boundary that have not been adequately addressed as required by Rule 6.4.19. <br />Please revise this Exhibit to include all applicable structures and utilities. <br />Please submit proof of publication per Rule 1.6.2(d) and proof that all owners of surface and mineral <br />rights to the affected area, and all landowners within 200 feet of the affected area have been properly <br />notified per rule 1.6.2(e). If any applicable property owners or structure owners (including utilities <br />companies) were not notified, please notify them per the above noted rule. The decision date for this <br />application will need to be extended to allow for adequate public comment period for those who were not <br />notified at the beginning of this conversion application process unless those entities sign a waiver of their <br />rights to the entire public comment period and indicate that they have no objections to the.application. <br />
