Reclamation Permit Amendment Application
Dear Mr. Goemmer:
On May 25, 1994, the Division of Minerals and Geology received your
application package for the Goemmer Pit, Permit No. M-82-129. The initial
submittal was to convert the pit from a 110(2) Limited Impact permit to a
112 Regular Reclamation permit. After Larry Oehler spoke to Mrs . Goemmer on
June 14, 1994, it was determined that a 110(2) Amendment application should
have been submitted instead of the conversion application . Preliminary review
of the information that you submitted as a conversion determined that you must
submit the following items before technical review of your application can
begin:
110(2) APPLICATION FORM: Please complete this form and return the
original and one copy to the Division (be sure to give a copy of this
application to the County Clerk and Recorder and ask them for a receipt
to submit to the Division to complete Exhibit I -,Proof of Filing with
the County Clerk) .
EXHIBIT A - Location Map: Please indicate on the USGS Quad map the
location of the mine.
EXHIBIT B(2) - Critical Structures within 200 feet and offsite damage:
See Page 5 - Exhibit B - Site Description for information necessary to
complete this exhibit.
EXHIBITS I and J: You will need to redo these two exhibits because of
the difference in permit applications.
You may amend your application by submitting the requested items . In
accordance with Rule 1 .5, a new decision date will be established 30 days from
the date of receipt of all requested items . If you have already initiated
publication, you will need to republish notice which specifies a 10 day public
comment period from the new date of newspaper publication.
