The decision date for your application is scheduled for October 17, 2002. <br />Therefore, I would appreciate a response from you on or before September 30, 2002. (Your <br />response should include two (2) copies). Any public comments that we receive on your <br />application during the interim will be forwazded to you. Please be advised that your conversion <br />application may be deemed inadequate and may be denied on October 17, 2002 unless the <br />following adequacy review items are addressed to the satisfaction of the Division. <br />6.4.2 EXHIBIT B -Index Maa <br />1. The Index Map provided is uncleaz. In conformance with Rule 6.4.2, please provide a clean <br />copy of an Index Map that shows the location of the pit in reference to either Milner or <br />Steamboat Springs. <br />6.4.3 EXHIBIT C -Pre-Mining and Mining Plan Map(s) of Affected Lands <br />2. Irr conformance with Rule 6.4.3, please provide a revised copy of Exhibit C which legibly <br />portrays the following information, as applicable: <br />(a} all adjoining surface owners of record; <br />(b) the name and location of all creeks, roads, buildings, oil and gas wells and lines, and power <br />and communication lines on the area of affected land and within two hundred (200) feet of all <br />
