An assessment conference was held at the Division's office on <br />December 5, 1994. In attendance at the conference were Dave Berry representing the <br />Division and Jerry Koblitz representing Slurco Corporation. If you agree with the penalty, <br />please sign the settlement agreement within ten (10) days as instructed on the form. If you <br />disagree with the settlement agreement, do not sign it. The Division will fix the penalty <br />pursuant to Rule 5.04.3(5) and you may request a Board Hearing.
