Following is the additional information which you requested:
NOV C-84-062, issued 4-26-84, failure to maintain and monitor surface water
monitoring stations. Sent to collection agency on 10-23-84 after our
administrative process was completed but no penalty payment had been
made. Fault was assessed at $250.00 and was not adjusted during the
assessment conference.
NOV C-84-063, issued 4-26-84, failure to construct emergency spillway in a
sediment pond. Also sent to collection agency as described above.
Fault was assessed at $750.00 (maximum for negligence) and was not
,7~is ed during the assessment conference.
NOV C-84-067~issued 5-3-84, for failure to pass disturbed drainage through a
sediment pond. Also sent to collection agency for payment as described
above. Fault was assessed and upheld at $750.00, maximum for
negligence.
fJOV C-84-157, Issued 9-7-84, for failure to maintain a collection ditch.
Penalty was fixed on 11-6-84, should be paid by 12-13-84. Fault /
component was $0.00. _
NOV C-84-164, issued 11-1-84, failure to reclaim contemporaneously. Proposed
penalty was sent out 11-30-84. Fault was assessed at $750.00, maximum
for negligence.
NOV C-84-165, issued 11-1-84, failure to maintain collection ditch. Proposed
penalty sent out 11-30-84. Fault was assessed at $250.00.
Currently we are holding a corporate surety bond for $58,073.00. We requested
that an additional $9,700.00 be submitted as a result of a stipulation
response for restablishment of a drainage diverted through the site. The plan
for reconstruction of ttre drainage was submitted in October 1983. Conditional
approval was granted in November 1983 pending submittal of the additional
