Preliminary review of the information <br />received determined that the following items must be received before the Division can consider your application as being <br />submitted and technical review can begin: <br />APPLICATION FORM: The following items from the application form require further information or <br />clarification. <br />Item no. 1, Applicant/operator name: Please be reminded that this conversion application is submitted under the <br />name of Denison Mines (USA) Corp., though the pernvttee is currently International Uranium (USA) Corp. The <br />succession of operator and transfer of permit from International Uranium (USA) Corp. must be completed and <br />approved before technical review of this application may commence. <br />Item no. 3, Permitted acreage: Please revise the acreage of the area to be added and/or the new total acreage so <br />that the sum is corrected. (T'he revised acreages may require revising the figures contained in the description of <br />the conversion application on page 4.) <br />Page 5: The Responsibilities as a Pernvttee must be completed and provided. <br />Page 6: The Note to Commentors/Objectors must be included in the application. <br />Page 7: The Certification must be completed, with signature, notarization and corporate seal. <br />EXHIBIT C -Pre-mining and Mining Plan Man(sl of Affected Lands (Rule 6.4.3): Please provide the <br />following information on one or more of the maps in this exhibit: <br />(a) all immediately adjacent surface owners of record; <br />(b) the designation or names of the owners of permanent roads (such as county roads, BLM- <br />managed roads, etc.), and owners of power and communication lines on the area of affected land and within two <br />Office of Office of <br />Mined Land Reclamation Active and Inactive Mines <br />
