On May 21, 2007, the Division of Reclamation, Mining and Safety received your 112 Hard Roclc/Metal Mining Permit
Conversion Application CN-I for the Topaz Mine, Permit No. M-1980-055 HR. Preliminary review of the information
received determined that the following items must be received before the Division can consider your application as being
submitted and technical review can begin:
APPLICATION FORM: The following items from the application form require further information or
clarification.
Item no. 1, Applicant/operator name: Please be reminded that this conversion application is submitted under the
name of Denison Mines (USA) Corp., though the pernvttee is currently International Uranium (USA) Corp. The
succession of operator and transfer of permit from International Uranium (USA) Corp. must be completed and
approved before technical review of this application may commence.
Item no. 3, Permitted acreage: Please revise the acreage of the area to be added and/or the new total acreage so
that the sum is corrected. (T'he revised acreages may require revising the figures contained in the description of
the conversion application on page 4.)
