Performance Warranty
Dear Mr. Onorofskie:
On July 27, 2007, the Division of Reclamation, Mining and Safety received a Succession of Operators Application, a
$125.00 application fee and a Performance Warranty for the Eagle-Gypsum Mine, Permit No. M-1984-041. The Division
is returning these documents for the following:
Succession of Operators ADUlication
The Division requires submittal of a Succession of Operators Application on an original board-approved form,
rather than a faxed copy of this document. Please fmd aboard-approved Succession of Operators Application
enclosed for your use.
The Division requires a replacement Financial Warranty be submitted when Option B is selected on page 1. If
you do not intend to submit a Financial Warranty at this time, please initial Option A.
On July 1, 2007, the Succession of Operator Application fee was raised from $125.00 to $144.00. Please submit
the supplemental $19.00 of the application fee.
Performance Warranty:
The Division requires submittal of a Performance Warranty on an original board-approved form, rather than a
faxed copy of this document. Please find aboard-approved Performance Warranty enclosed for your use.
