I have completed an initial check of the AVS files for the subject transfer. As we discussed, I have
initiated the transfer action.
The following information or clarifications are still required (these are identical to those requested
in the reference memorandum). In this memorandum, documentation means minutes of board
meetings, letter of resignation, letters of assignments, contracts, etc.
With regard to Lodestar Energy, Inc., the AVS database carries the begin date for R. Eberley Davis
as Vice President as 11/14/1995. The information in the submittal (submittal) is 09/0295. Please
provide documentation for the correct date. The first date of appointment of uninterrupted service
is the date that should be used.
