An assessment conference was <br />held at the Division's office on October 29, 1993. In attendance <br />at the conference, in addition to yourself and me, was Steve <br />Shuey representing the Division of Mlnerals and Geology. If you <br />agree with the penalty, please sign the settlement agreement <br />within ten (10) days as instructed on the form. If you disagree <br />with the settlement agreement, do not sign it. The Division will <br />fix the penalty pursuant to Rule 5.04.3(5) and you may request a <br />Board Hearing.
