M-84-097, RECEIPT OF ll2 <br />CONSTRUCTION MATERIALS RECLAMATION PERMIT AMENDMENT APPLICATION <br />PACKAGE-INADEQUATE <br />Dear Mr. Rogers: <br />On June 28, 1996, the Division of Minerals and Geology received your application package for <br />a ll2 Construction Materials Reclamation Permit Amendment for the above referenced <br />operation. Preliminary review of the file and documentation received determined that this <br />operation is an existing Limited Impact 110 Operation and the wrong type of revision was <br />requested. <br />According to the information you provided and the findings of Steve Shuey in his inspection <br />report dated September 11, 1995, this application should be a conversion application and not an <br />amendment. Therefore, the appropriate fee for this conversion is $1,875. You submitted, along <br />with your application package, a check in the amount of $1,550. Please submit the additional <br />$325 so that we may begin the completeness review process. <br />This letter shall not be construed to mean that there are no other technical deficiencies in your <br />application.
