This date is 180 days <br />prior to expiration of the existing five year permit on April 21, 1988. <br />The minimum requirements for renewal applications are described in <br />Rule 2.08.5(2) of the regulations. Please find enclosed an application form <br />to be completed and submitted with the application. The application should <br />also include any revised pages or additional information required to update <br />the current permit document.
