I have enclosed the <br />form that is referred to in 2.08.6C2)(b). I have also enclosed an example of <br />the publication that 1s required under 2.08.6(3)(a). <br />Basically, you have to fill out the enclosed form as per Rule 2.08.6(2)(b). <br />Please note the information needed per Rule 2.08.6(2)(b)(iti>. The <br />performance bond needs to be obtained under the new name, as per Rule <br />2.08.6(2)(a). The proposed change needs to be advertised, per Rule <br />2.08.6(3). After the 15 day comment period passes, as mentioned in <br />2.08.6(3)(b), the Division can give approval for the name change provided that <br />the provisions in 2.08.6(4) and 2.08.6(5) are met. A formal hearing can be <br />requested under 2.08.6(6) within 30 days after notification by the Division of <br />its decision. <br />An AVS check (applicant violator system) will be performed to search for any <br />bond forfeitures or other unresolved obligations incurred in other states. <br />
