1,l <br />Recommended Conditions for Continued Operation of (~'~ y <br />~;~'~ `~ Z <br />The Battle Mountain Gold Ore Processing Plant ,-~;. ~y f~Qv/'T~_ <br />a <br />Permit M-88-112 ~rJ'r <br />(1) lower the WAD Cya~i de levels i ~ the Tai 1 i ngs Impoundment to 50 ~ ppm <br />BMG stated in their Hydrogen Peroxide Tailings Treatment TR-00 <br />application; "Existing WAD cyanide concentrations in the taili gs <br />impoundment will be reduced to less than 50 ppm... Peroxide w 11 <br />continue to be added directly to the liquid pools until the WA cyanide <br />concentration drops below 50 ppm." <br />If BMG has~no achieved tai ings impoundment WAD cyanide level of 50 ppm <br />or less y July 3, 1992, B G shall immediately notify the Divi ion and <br />cease operation of the s~ e processing plant until it has achie ed the <br />reduce WAD con~t-~-a~ions. <br />(2l Maintain the Tailings Impouadme~t WAD Cyanida Levels At or °eldw 50 ppm <br />After July 3, 1992, if the WAD cyanide levels within the taili~gs <br />impoundment rise above 50 ppm, BMG shall immediately notify th Division <br />and cease operation of the ore processing plant until the WAD yanide <br />levels have returned to 50 ppm or lower. <br />(3) Implement the Ground Water Monitoring Plan Modifications <br />Immediately following the Division's approval of technical revision <br />application TR-008, BMG shall submit well permit applications f r the two <br />proposed additional monitoring wells. Further, BMG shall conta t Mr. <br />George Van Slyke of the Division of Water Resources to request erbal <br />approval to proceed with installation of the two wells. The Di ision <br />will request that DWR expedite approval of these well permits. <br />Within thirty days following receipt of verbal approval from th' DWR to <br />install the monitoring wells, BMG shall install, complete, hydr logically <br />test, geophysically log, and sample the wells. BMG shall also valuate <br />the data and submit a report presenting BMG's conclusion's rega ding the <br />necessity of additional ground water monitoring modifications. , <br />Within thirty days following the Division's approval of the evalIuative <br />report submitted in accordance with TR-008, BMG shall install ahd <br />complete any necessary additional ground water monitoring wells; ~et~~ <br />If BMG fails to meet any of the above deadlines, or any leakage from the <br />tailings impoundment, BMG shall immediately notify the Division and cease <br />operation of the ore processing plant. <br />6-1~ 9z <br />
