Baxter / Executive Director <br />Midway Aggregates, RLLP / Ronald W. cottony <br />P O Box 580 Division Director <br />Rye, CO 81069 Natural Resource Trustee <br />./~ l~l~ kU~ ~--- <br />Re: Midway Pit, Permit No, M-198&018, Receipt of Constriction Materials Reclamation Permit <br />Amendment Application Package, AM - 1 `~~ <br />Dear Mr. Baxter: <br />On August 28, 2006, the Division of Reclamation, Mining and Safety received your request for a partial acreage release <br />and transfer of acreage location. in order to review your request adequately, the Division is requesting the submittal of a <br />112 Construction Materials Reclamation Permit Amendment Application package for the Midway Pit, Permit No. M- <br />1988-018. Please find this application package enclosed for your use. <br />The following items must be received before the Division can consider your application as being submitted and technical <br />review can begin: <br />APPLICATION FEE: The Division must receive a $188.00 application fee with the application package. <br />APPLICATION FORM: The Division requires completion of the enclosed application form. <br />EXIIIBIT A -Leval Descrindon (Buie 6.4.D: The legal description must identify the affected land and be <br />wholly adequate for field location of the property. It can be in the form of metes and bounds survey or a <br />description by reference to township, range and section to at least the nearest quarter quarter section. Where <br />applicable, the street address or lot numbers maybe used. <br />EXIIIBIT B -Index Map (Rule 6.4.2): An index map showing the regional location of the affected land and all <br />roads and other access to the azea. A standazd U.S. Geological Survey topographic quadrangle or equivalent is <br />acceptable. Scale criteria need not be followed for this map. <br />EXHIBIT C -Pre-minine and Mining Plan Mao(sl of Affected Lands (Rule 6.43): One or more maps may <br />be necessary to legibly portray the following information: <br />(a) all immediately adjacent surface owners of record; <br />(b) the name and location of all creeks, roads, buildings, oil and gas wells and lines, and power and <br />communication lines on the area of affected land and within two hundred (200) feet of all boundaries of such <br />area; <br />(c) the existing topography of the azea with contour lines of sufficient detail to portray the direcfion <br />and rate of slope of the affected land; <br />Office of Once of <br />Mined Land Reclamation Active and Inactive Mines <br />
