M-84-097, RECEIPT OF 110 TO ll2 CONSTRUCTION <br />Dear Mr. Latour: <br />On June 29, 1996, the Division of Minerals and Geology received your 110 to 112 Construction Materials <br />Reclamation Permit Conversion Application package for the Mineral County Pit, Permit No. M-84-097. <br />Preliminary review of the information received determined that the following items must be received before <br />the Division can consider your application as being submitted and technical review can begin: <br />1) APPLICATION FEE: The Division did receive the $325 balance of the required $1,875.00 <br />application fee. Please excuse the delay in responding, the receipt was misfiled and did not come <br />immediately to my attention. <br />2) Application Form: In item N3, Permitted Acreage, it is not cleaz from the form what is the amount <br />of the existing original permitted acreage and how much additional acreage will be added by this <br />conversion. <br />3) EXHIBIT A -Legal Description (Rule 6.4.1): The legal description must identify the affected land <br />and be wholly adequate for field location of the property. It can be in the form of metes and bounds <br />survey or a description by reference to township, range and section to at least the nearest quarter <br />quarter section. Where applicable, the street address or lot numbers may be used. In the case of a <br />conversion application, the legal description must be provided to reflect the changes. <br />4) EXHIBIT O -Proof of Mailing of Notices to Count~Commissioners and Soil Conservation District <br />fRule 6.4.17): Please submit proof of actual delivery or proof of mailing by Certified Mail, return <br />receipt requested, of Notice of the Application to the local Board of County Commissioners and, if <br />the mining operation is within the boundaries of a Soil Conservation District, to the Board of <br />Supervisors of the Soil Conservation District. Initial proof of mailing, for completeness, is a copy <br />of the white receipts for such mailings. <br />
