Proof of notice may be return receipts
' of a Certified Mailing or by proof of personal service.
Response: See Attachment #1-Proof of publication of the Public Notice and proof
' of notice to all owners of record of surface lands and easement holders
within 200 feet of the affected area of Permit Amendment 006.
Comment: Exhibit C-1 depicts structures located within the permit azea. However,
the area outside of the permit area (south of the permit azea along
Hygiene Road, southwest of the foothills inlet and west of Foothills
Highway) is not depicted on the Exhibit. Please revise
the map accordingly.
Response: Exhibit C-1 shows structures located within the amendment azea as well
as structures within 200 feet of the affected azea including the azea west
of Foothills Highway. Regazding the area south of Hygiene Road east of
North 61st Street, the two structures not shown on Exhibit C-1 aze the
Cargill and Andrews groundwater wells. However, these wells are
identified on Exhibit "O"-Adjacent Surface Owners Map, which is part
' of the original amendment application submitted to the Division on July
1, ] 999. In addition, Attachment #2-Aerial Blueline shows all
structures adjacent to the proposed amendment boundary.
~~ 3.
