Comment: As required by Rule 1.6.5 (20, please submit the proof of publication of <br />the public notice circulated in the local newspaper and all owners of <br />a record of surface lands, and easement holders that are within 200 feet of <br />the boundary of the affected land. Proof of notice may be return receipts <br />' of a Certified Mailing or by proof of personal service. <br />Response: See Attachment #1-Proof of publication of the Public Notice and proof <br />' of notice to all owners of record of surface lands and easement holders <br />within 200 feet of the affected area of Permit Amendment 006. <br />Comment: Exhibit C-1 depicts structures located within the permit azea. However, <br />the area outside of the permit area (south of the permit azea along <br />Hygiene Road, southwest of the foothills inlet and west of Foothills <br />Highway) is not depicted on the Exhibit. The Division needs to <br />determine what is surrounding the permit azea, specifically, permanent <br />man-made structures within 200 feet of the affected azea. Please revise <br />the map accordingly. <br />Response: Exhibit C-1 shows structures located within the amendment azea as well <br />as structures within 200 feet of the affected azea including the azea west <br />of Foothills Highway. Regazding the area south of Hygiene Road east of <br />North 61st Street, the two structures not shown on Exhibit C-1 aze the <br />Cargill and Andrews groundwater wells. However, these wells are <br />identified on Exhibit "O"-Adjacent Surface Owners Map, which is part <br />' of the original amendment application submitted to the Division on July <br />1, ] 999. In addition, Attachment #2-Aerial Blueline shows all <br />structures adjacent to the proposed amendment boundary. <br />~~ 3. Comment: Rule 6.4.3(g) requires the applicant to show the owner's name of <br />structures within 200 feet of the affected land. Please indicate the <br />owners of all structures located inside the affected area and within 200 <br />feet of the affected azea. Such structures as defined by Rule ].1(48) <br />include buildings, houses, barns, fences. above or below ground utilities, <br />imgation ditches, maintained or public roads, bridges, railroad tracks, <br />cemeteries, communication antennas, pipelines, water wells, water <br />storage impoundments, discharge and conveyance structures, etc. If any <br />structures will be removed or relocated, please state that fact in the <br />DMC Adequacy Review Response Page I <br />
