Bama, <br />Division Director <br />After further review of Salt Creek Mining Company's application for <br />a permit revision and successive renewal at the McClane Canyon <br />Mine, two additional adequacy concerns have been raised. These <br />concerns will need to be addressed before the Division can issue a <br />decision on the application. <br />1. The insurance certificate submitted to the Division by Salt <br />Creek Mining Company contains the phrase "Should any of the <br />above described policies be cancelled before the expiration <br />date thereof, the issuing company will endeavor to mail 30 <br />days written notice..." Please have the policy amended such <br />that the issuer of the certificate commits to providing the <br />Division with 30 days written notice of cancellation. The <br />certificate also specifies both the McClane Canyon Mine and <br />the Munger Mine, as requested, but does not specify the amount <br />of coverage for each mine. The certificate should state that <br />each operation has in force the minimum coverage requirements <br />specified in Rule 2.03.9(1). <br />2. Please submit Cultural and Historic Resource Information for <br />the new permit area, as required by Rule 2.04.4. <br />STATE~F COLORADO <br />
