Identification of Interest Change; AVS
Kent, I updated the owners and controllers for Picketwire Processing LLC in the AVS, and the
system recommendation was `issue.' I do have a few comments that need to be addressed by the
operator;
The operator provided the Division with a certified corporate resolution to withdrawal
A.P. Maxwell as a shareholder, and remove A.L. Perry and Bruce Addington as officers
of Picketwire Processing LLC. The corporate resolution did not state if A.L. Perry and
Bruce Addington aze still directors of Picketwire. If they aze directors, they need to be
included in the permit pages. If they aze not, then the termination needs to be included in
the resolution.
2. A number of officers for Picketwire aze listed in the AVS, but aze not included in the
permit pages. They are the following;
Yoichi Takenaka- Director/Manager
Koichiro Inami- Director/Manager
John Patterson- Director/Manager
If the above-mentioned officers and directors for Picketwire are correct, then they need to
be included in the permit pages. If they are no longer associated with Picketwire, then
proof of resignation in the form of corporate minutes or certified statements is required.
