Letter to U.S. Soil Conditioning Company <br />Re: Notice of Violation MV-2006-050 <br />August 11, 2006 <br />page 1 <br />IN ORDER TO AVOID POSSIBLE PERMIT REVOCATION AND BOND FORFEITURE, PLEASE <br />SUBMIT $688.00 FOR ANNUAL FEE5 DUE AND $688.00 FOR CIVIL PENALTIES ASSESSED (FOR A <br />TOTAL OF $1,376.00, BY September 11, 2006. <br />The Boazd Meeting will be held on October 18, 2006 (or the following day), in Room 318 of 1313 Sherman Street, <br />Denver, Colorado, beginning at 9:30 a.m. or as soon thereafter as the matter can be considered, and you will have the <br />opportunity to be represented and heazd. <br />Following the hearing, all parties have an opportunity to review and comment on a draft of theBoard's arderbefore <br />it is issued. If you wish to do so, you must provide the Board a written request to review the draft order at your <br />hearing. You must include your mailing address and, if possible, a fax number ar email address. You will be <br />provided a copy of the draft order for your review as soon as it is available. The Board must receive any comment <br />within five days following your receipt of the draJl order, unless [hat deadline is extended. Submit all comments in <br />writing to Tyson Powell, Assistant Attorney General, 1525 Sherman Street, S`" Floor, Denver, CO 80203, FAX # <br />(303) 866-5395. The Board will resolve any issues regarding the form of the order. This process does not affect you <br />right to request reconsideration by the Board or appeal the substance of the order. <br />Please contact the Board's secretary, Ms. Glenda Williams, at the DRMS Denver Office, telephone 303-866-3929, to <br />confirm the specific date for the hearing. The Boazd's agenda may not be finalized prior to the week of the Board <br />meeting. Therefore, we recommend that you contact Ms. Williams on (Monday before Boazd meeting date).
