This <br />revised page addresses Item 1 from the Division's memo. <br />The AVS system was checked by Jim Lunan in PHCI's corporate <br />office in St. Louis for Item 2 from the Division's memo. OSM <br />is making the corrections on the end dates for Ryan Tew (PCC- <br />SEC) and Leslie Ashford (PLC-DIR). The end dates are identical <br />to those I submitted on November 3, 1995. The other <br />individuals listed in the Division's memo (Wimberly-HM <br />Holdings, VP and PLC, DIR; Hellings, Harper, and Chiese-PLC, <br />DIRS) are consistent between OSM and the listing which I sent <br />on November 3, 1995.
